Extended Care Center modeled after a Florida program. The Prescribed

- In September of 1993 PPC was opened, the first program of its kind in
Toledo.

- Small Groups – Staff members are assigned only three to four children.
- PPC is specifically designed to care for children with medically complex
needs.
- PPC provides skilled nursing services with a staff of RNs and LPNs.

Therapists, Early Intervention staff, and Case Managers to promote
healthy growth and development.
- Collaboration – PPC has strong long term collaborative relationships
with the Lucas Co. Board of DD, Toledo Public School, and the Early
Learning Center (a NAEYC accredited childcare program)

- PPC is open all year. PPC is only closed six days all year. New Years
Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving Day and the
day after Thanksgiving, and Christmas.

- Prescribed Pediatrics is staffed by RNs, LPNs, STNAs, and Licensed
Teachers.
- At least one RN is on site at all times.
- PPC’S Medical Director is a renowned Pediatrician and Professor Emeritus
at the University of Toledo.
- PPC’s Manager has over 10 years experience working with families and developmental disabilities.
- PPC’s Nursing Supervisor has nearly 20 years of experience in the
medical field.
- PPC’s Childcare Coordinator is a graduate of BGSU’s Early Childhood
Education program.
- All PPC Staff receive at least 10 hours of training in Early Childhood Education
each year.

- Pick up the phone – referrals can be made by families, case managers,
physicians, early intervention specialists, NICU and PICU personnel,
etc..
- Contact the PPC Manager at 530-6734 or the Nurse Supervisor at 530-6726.
- Visit PPC on the web at www. prescribedpediatric.org and fill out an
information request.

- When calling for a referral it is helpful to have the following
information ready:
- Name and D.O.B of the child
- All medical diagnosis.
- List of physicians.
- List of medications.
- List of special needs the child(ren) may have during childcare.
- Name of Insurance companies that are covering the child including along
with the plan number and names of case managers.

- PPC Staff will review the information with the Manager, Director of
Health Services, and the Medical Director to ensure Prescribed
Pediatrics is appropriate.
- The Family will next be asked to complete an intake packet. PPC staff is
available to assist families to complete the packet.
- A funding source must be identified and agreed upon by all parties prior
to services beginning.
- All physicians orders must also be sent to PPC before admission to the
program.

- Fees vary depending on the services being provided.
- PPC works with families to explore funding opportunities that may be
available to them.
- Most families find PPC rates to be very competitive with other childcare
program.
- Common funding sources include:
- Private Insurance
- Medicaid
- Waivers
- Ohio Department of Jobs and Family Services
- Private Pay
- Help Me Grow Respite Funds
- Scholarships (Short term, generally 90 days, while additional funding
sources are being identified).

- Parents are responsible for providing transportation to and from PPC.
- Insurance or waivers may pay for transportation services to and from
PPC.
- In certain situations when the child is also enrolled in the Toledo
Public School classroom located in the EduCare building the school
district may provide for part of the transportation.

- All medications needed for the day.
- Staff must have a physician’s order to administer medications.
- Medications must be in the original container with the label from the
pharmacy.
- All meals including bottles and formula.
- Diapers and wipes.
- Change of clothes.
